Using skepticism, I would review the different methods used in studying the disease, preventing and treating it and assess their effectiveness (Oermann et. al., 2000). Advanced Nursing practice course has taught me how to utilize nursing theories in interpreting and understanding a situation. For example, I have found Callista Roy’s Adaptation Theory useful in understanding disease and illnesses especially because it presents the human person in a holistic manner. As a nurse educator, I can use this theory in instructing a nurse on how to assess a patient on the basis of its four adaptive modes namely, physiologic, self-concept, role function and interdependence in deciding on the intervention that addresses all the needs of a patient. History and Physical Examination Essay

Admitting Diagnosis: Stomatitis possibly methotrexate related. Chief Complaint: Swelling of lips causing difficulty swallowing. HISTORY OF PRESENT ILLNESS: This patient is a 57-year-old Cuban woman with a long history of rheumatoid arthritis. She has received methotrexate on a weekly basis as an outpatient for many years. Approximately two weeks ago she developed a respiratory infection for which she received antibiotics and completed that course of antibiotics. She developed some ulcerations of her mouth and was instructed to discontinue the methotrexate approximately 10 days ago. She showed some initial improvement but over the last 3 to 5 days has had malaise, a low grade fever and severe oral ulcerations with difficulty in swallowing. Although she can drink liquids with less difficulty. Patient denies any other problems at this point except for a flare of arthritis since discontinuing the methotrexate. She has rather diffuse pain involving both small and large joints. This has caused her some anxiety. MEDICATIONS: 1. Prednisone 7.5 mg p.o. daily. 2. Estradiol 0.5 mg p.o. q.a.m. 3. Mobic 7.5 mg p.o. daily, recently discontinued because of questionable allergic reaction. 4. HCTZ 35 mg p.o. every other day and oral calcium supplements. 5. In the past she has been on penicillamine, azathioprine, and hydroxychloroquine but she has not had Azulfidine, cyclophosphamide or chlorambucil. ALLERGIES: None by history. FAMILY/SOCIAL HISTORY: None contributory. PHYSICAL EXAMINATION: This is a chronically ill appearing female alert oriented and cooperative. She moved with great difficulty because of fatigue and malaise. VITAL SIGNS: Blood pressure 107/80. Heart rate 100 and regular. Respirations 22. HEENT: Normocephalic, no scalp lesions, dry eyes with conjunctival injection, mild exophthalmos, dry nasal mucosa, marked  cracking and bleeding of her lips with erosions of the mucosa. She has a large ulceration of the mucosa at the bite margin on the left. She has some scattered ulcerations on her hard and soft palate. She has difficulty opening her mouth because of pain. Tonsils not enlarged. No visible exudate. SKIN: She has some mild ecchymosis on her skin and some erythema. She has patches but no obvious skin breakdown. She has some fissuring in thebuttocks crease. PULMONARY: Clear to percussion and auscultation bilaterally. CARDIOVASCULAR: No murmurs or gallops noted. ABDOMIN: Soft, non-tender, protuberant, no organomegaly and positive bowel sounds. NEUROLOGIC: Cranial nerves 2 through 12 are grossly intact. Diffuse hyporeflexia. MUSCULOSKELETAL: Corrosive destructive changes in the elbows, wrists and hands consistent with rheumatoid arthritis. Has bilateral total knee replacements with stove pipe legs and perimalleolar pitting edema 1+. I feel no pulses distally in either leg. PHYCIATRIC: Patient is a little anxious about these new symptoms and theyre significance. We discussed her situation and I offered her psychological services. She refused for now. PROBLEMS: 1. Swelling of lips and dysphasia with questionable early Stevens-Johnson syndrome. 2. Rheumatoid arthritis class 3, stage 4. 3. Flare of arthritis after discontinuing methotrexate. 4. Osteoporosis with compression fracture. 5. Mild dehydration. 6. Nephrolithiasis 7. Anxiety PLAN: 1. Admit patient for IV hydration and treatment of oral ulcerations. 2. Obtain a dermatology consult.  3. IV leucovorin will be started and the patient will be put on high dose corticosteroids. 4. Considering patients anxiety perhaps obtain services of Stella Rose Dickinson PHD phycology at a later date.

The article proposes that the Church could be utilized as a similar model in which it serves as Forgiving Community in which all levels of leadership would cater to the community from infancy through adulthood with various types of programing and education. Personal Response I was interested in this article because I know how detrimental it can be to hold onto forgiveness. Throughout my 18 years of service in the U.S. Air Force, multiple deployments and several assignments in leadership, I have witnessed how holding onto past wrongs can eat away at you like a cancer and often time, it goes unnoticed until truly identified and examined by self-identification or through third party intervention. I have been involved with several situations in which members deploy into a combat zone and return different people. Many have been wronged by a common enemy and struggle to even examine the idea of forgiveness. Many soldiers carry around this pain and baggage for years without ever truly recognizing exactly what they are holding onto. The Church can be vital to this recognition and the recovery process. The article relayed how the Church can play such a vital role as a Forgiving Community reaching to all members of the family from child to adult. It was also interesting how among the various levels of leadership and roles in the Church, each one was able to offer their own gifts and talents providing to the community. It relays how a community must be all-encompassing feeling of safety and opens not only to give forgiveness, but to accept forgiveness as well. I would like to further investigate this topic by researching small group studies on forgiveness that are available through my own local Church community. I am part of a small group that meets regularly and would like to incorporate the topic into our lessons. In addition, I see that Robert Enright has done a vast amount of research on the topic and has produced many works as a result. One of the books that I would like to read is his Forgiveness is a Choice: A Step-by-Step Process for Resolving Anger and Restoring Hope (2001). I understand that it is something that we must decide to do. Until we make the first step to accept and be willing to forgive, or be forgiven for that matter, we will bear the ever increasing weight of the wrong and carry it with us throughout our journey through life darkening our  outlook and damaging our hope. Application The setting for the application of the information in this article would be the Aviano Center, a small non-denominational Church located just outside of the Aviano Air Force base in Pordenone Italy. The client who came in for our session was a 30 year old wife and stay-at- home mother of two toddlers whose husband was currently deployed to Afghanistan. They had lived in the area for about 4 months before he deployed and he is currently 5 months into an 8 month deployment. She states that she can no longer deal with the children. She has found herself trying to cope alone and often finds herself losing her patience and yelling at the children. She is afraid that she is going to end up just like her mother, who she resents because of the verbal and physical abuse that she had put her through when she was a child. She is ashamed and embarrassed about her situation. She approached me because I am one of the leaders at Aviano Center and she knows that I am in the military and also the â€Å"Life Group† facilitator who organizes the small groups for our Church. In addition, the group that I host is particularly for those families whose family member is about to deploy, is deployed or recently returned from a deployment. In the meeting with this member, I would attempt to get her to realize that she is potentially dealing with several issues, with forgiveness and resentment potentially playing a major role in them. She may be holding on to past wrongs committed by her mother and past and present wrongs that she has committed herself. I would walk her through the forgiveness process and highlight that it is a learned trait that must be practiced. I would explain that forgiveness involves both granting and receiving forgiveness. I would explain the forgiveness models, the details of those models and explain that the Church can be a model of a forgiveness community. Additionally, I would reiterate that she is not alone in feeling the way she does, in fact I would offer that there are many who feel similarly right within the Church making her aware of the current small group focused on the facilitation of the forgiveness process. I would encourage her that these groups are a caring  community that respects confidentiality. Regardless of where the conversation led, in closing, I would ask her if she would like to pray with me. I believe that God has enabled us with this ability to communicate to encourage, provide hope and comfort in situations just like this. Artistic Expression in 18th and 19th Century America The first settlers in the New World faced unpredictable hardships. The men of the Virginia colony had enough trouble learning to live off the land, let alone having to defend themselves from native attacks. Famine proved to be a hard obstacle to conquer for all of the new colonies. New England, while having a more suitable climate for the prevention of diseases, also had its conflicts with local tribes. The Puritan ideals of New England were very strict especially in regard to private indulgences, including art. The only type of art that was "acceptable", were portraits, almost exclusively of upper class citizens and clergy. Through the years leading up to the revolution, however, as the population became diversified, new ideas started to influence popular thought. There were many different cultures coexisting as well as different social classes. However, as was the standard in Europe, only the upper class people were part of the "art world". Look at paintings from the per revolutionary era, including The Mason Children: David, Joanna, Abigail, (unknown artist), Hanna Minot Moody (Joseph Badger), New England Merchant (Charles W. Peale), Portrait of Elizabeth, The Artist's Daughter (John Singleton Copley), we notice many similarities among them. As stated previously, they are all portraits. All of the subjects are portrayed in very fine clothes and are obviously posing for the painting. Most of these works have little or nothing at all in the background. One gets a very "cold and sterile" feeling when looking at these, and they are most certainly intended not for artistic expression but simply to record the image. "As late as 1800, owing to the limited economy of the Colonies as well as the Puritan’s prejudice against idolatry and their regard for art as a luxury, portraiture was the only accepted form of expression in painting." (Bazin 341). In the period following the American Revolution up to the turn of the eighteenth century, we start to see some subtle yet significant changes in the popular artwork. We can see a much bolder use of color and background, as in C.W. Peale’s Benjamin and Eleanor Ridgely Laming (1788) and Mather Brown’s Sir Richard Arkwright (1790). Emotion is shown of the faces of the subjects and Ralph Earl showed the Striker Sisters (1787) with their arms around each other and pleasant, happy smiles on their faces. Mr. Peale’s work shows the physical attraction between Benjamin and Eleanor.

Destiny, Fate, Free Will and Free Choice in Oedipus the King - Power of

The Power of Fate in Oedipus Rex (the King) The underlying theme in Oedipus Rex is that fate is more powerful than free will. On this strong basis of fate, free will doesn't even exist. This was a popular belief among the ancient Greeks.   Fate may be accepted or denied by modern society, but in Oedipus's story, fate proves inevitable. In the play, Oedipus Rex, the characters Oedipus, Iocaste and Laios try to change fate.    In the very beginning of the story, before we hear from the oracle, there is already foreshadowing of Oedipus' impending doom. He, himself, states to the people, "Sick as you are, not one is as sick as I" (Sophocles 5). This statement is almost eerie when looking back upon it. Alone, it seems as if he knows that he is ill fated, but reading on he clarifies his pain in this way:    Each of you suffers in himself alone His anguish, not another's; but my spirit Groans for the city, for myself, for you" (Sophocles 5).    His pain is not his future; it is the plague of the country. Oedipus was told by Teiresias that in his later years he would be the killer of his own father, and would marry his own mother. In his attempt to avoid this situation, he left both of his parents and traveled to a far away city called Thebes. Once there he was married to a woman, that he, himself, was positive was not his mother, for his mother was the woman that he had left back in Corinth. Also, being so far from his known home, there was no chance that he could kill his father whom he had also left behind. Oedipus thought he was safe, but he was not.    Oedipus is not the only one that tries to escape the curse. Iocaste also tried to escape the curse. Locke and Hobbes

What is politics? Merriam-Webster dictionary defines it as â€Å"the total complex of  relations between people living in society.† This phrase is the root of all politics and all  government. Whether we are ruler or subject, in the end we are all just â€Å"people.† And how can  Ã¢â‚¬Å"people† best relate to each other? Over the years, countless individuals believed that they  possessed the magic answer to this age-old question. Two figures in particular have endured the  praise and the criticisms: Thomas Hobbes and John Locke. Each man represents a philosophy  that has marked him in history and in the popular consciousness. And each man offers his  unique commentary—along with some surprising commonalities—on this abstract word we call  politics. How should an affective government approach its duties and functions? John Locke and Thomas Hobbes did hold some similar viewpoints on government’s responsibilities. Both men favored a social contract between government and its people (Baker, 2000). Hobbes stated in Leviathan, â€Å"Whensoever a man transferreth his right, or renounceth it, it is either in consideration of some right reciprocally transferred to himself, or for some other good he hopeth for thereby.† In other words, a man must be willing to sacrifice some of his basic rights in exchange for a promise of security and stability from his government. Ever since Moses and Biblical times, Hobbes points out, Man has operated on a contract system (Panagia, 2003). Likewise, John Locke also advocated this precursor to federalism (Baker, 2000): â€Å"men, when they enter into society give up †¦ liberty of a kind; yet it being only with an intention in every one the better to preserve himself, his liberty and property†¦.† (Locke, 2001) Locke believed that entering into such a covenant with government officials would best serve a â€Å"common good† and serve to create a â€Å"common law†Ã¢â‚¬â€ideals which were also shared by Hobbes (Baker, 2000). Hobbes himself claimed that â€Å"the point of the social contract is to orchestrate a multitude into some recognizable whole†¦.† (Hobbes, 1968). The government was dependent on the support of the people, and its â€Å"legitimacy†¦stable only as long as the public continues to recommend it† (Hobbes, 1968). Once the function of a government is determined, the next question becomes, â€Å"Who is the  government?† On this point, Hobbes and Locke part ways. Since Hobbes held that human beings were essentially â€Å"born bad,† then logic would follow that they are not fit to rule themselves.   Instead, they need â€Å"an overarching power to awe them†Ã¢â‚¬ ¦..likened to a sea monster, or a â€Å"leviathan† (Geib, 2002). Hobbes argued for a single-person leadership by claiming that multiple voices of authority created â€Å"too much diffidence† (or difference). Such confusion would inevitably plunge mankind back into a primitive state of warfare, the very condition it sought to eradicate through laws and government (Kreis, 2005): â€Å"for powers divided mutually destroy each other† (Hobbes, 1968). Hobbes had witnessed firsthand the conflicts between the English king and Parliament, which hurled that country into a state of near-civil war (Chodorow, 1994). War and conflict resulted from every man’s universal need for power. When one puts such strong egos into one room, resolution can never hope to be obtained. To Hobbes’ thinking, an entrustment of the power to one individual was the best way to ensure stability. And a monarchy, with its fixed and non-debatable succession of rulers, was Hobbes’ preferred choice of government (Panagia, 2003). Although Hobbes anticipated such democratic mantras as â€Å"the right to remain silent† and the â€Å"right to property,† his association with democracy affectively ends there. His absolute monarchy refutes the people as a sovereign power, and his concerns for individual liberty are miniscule (Gray, 2003).   In fact, Hobbes’ conservative views infuriated John Locke to such a degree that he published his own Treatises of Government, advocating a public-run government based on the liberty of the citizen. Locke raged that Hobbes’ monarchy left ruler and subjects in the very state of turmoil it proclaimed to avoid (Chodorow, 1994). A singular government could only serve to oppress and deny. Government should instead divide itself into branches, each serving its own specialized function while keeping the other branches from becoming too powerful (like the executive, legislative, and judicial branches of American government today). (John Locke, 2001) After all, Locke claims, the people cannot sacrifice more power than they possessed in their natural, pre-law state. Once the people enter into a social contract with their chosen government, then the sovereignty necessarily rests with the people (Landry, 1997). The people declare their leaders, and decisions rest with the â€Å"consent of the majority, giving it either by themselves or their representatives chosen by them† (Locke, 2001).   The government may tax, it may allocate funds based on need (Landry, 1997), but it â€Å"can never have a right to destroy, enslave, or designedly to impoverish the subjects† (Locke, 2001). Most crucial in Locke’s philosophy, the people rule. In the end, we are left with the question, â€Å"Whose philosophy is best?†Ã‚   Give us another  thousand years, and we will still probably be quarrelling about the answer.   Perhaps it is best   instead to let each man have his final say on the subject and leave it at that: â€Å"During the time men live without a common power to keep them all in awe, they are in that conditions called war; and such a war, as if of every man, against every man.†Ã¢â‚¬â€(Hobbes, 1968) â€Å"It is a power that hath no other end but preservation, and therefore can never have a right to destroy, enslave, or designedly to impoverish the subjects..†.—(Locke, 2001) References Baker, W. Concept of Love Essay

Love is a complex emotion of attachment and sometimes obsession. It cannot be classified so easily, though; it cannot be stuffed into a social concept and left there. Some people search for what seems their whole lifetime for love, whereas others may seem to fall in and out of love on a regular basis. Of course, people can love many things, but the love between two people is unique and special. It is a feeling of trust and wholeness. Love is rarely defined because everyone experiences it differently. What one person thinks is love, another may think is craze or vice versa. How does one know he or she is in love? How is it possible to distinguish between the love of romance and the love shared between friends? In order to understand love, people need to first understand the two major types of love which can be described as romantic love and friendship love. People often define romantic love based on its difference from friendship love. Would you kiss your best friend? Perhaps, but it is not necessarily the things you do with your romantic partner that makes your love spark. Romantic lovers are usually on each other’s minds all the time. Couples make large life decisions based on their romantic partners because they cannot imagine life without those people. Romantic love guides a person’s mind and can sometimes cloud it as to what the right decisions might be. After all, romantic love can be extremely selfless. Friendship love can often turn into romantic love, as it has many times throughout history, but the birth of romantic love between friends will often kill the love of friendship. In other words, friends who become romantic lovers rarely can go back to being â€Å"just friends. Why is it that romantic love is so overpowering? The love of friendship can be very strong, but it is the emotional intensity of romantic love that forever changes the simplicity of the love between friends. Friendship love does not have all the same guidelines that romantic love seems to have. Each friend couple may have a unique kind of love between each other. Friendship love is best described as a warm feeling between people. Girlfriends may have different friendship love for each other than a male and female friend pair may have; the same for male friends. Male and Female friends may sometimes undergo a lot of pressure from other friends and family to turn their friendship love into romantic love, which doesn’t always work. There is a line that friends may cross over, and to know that line is to know the difference between romantic and friendship love. Friends have complete trust for someone and naturally want to be around him/her. People who love each other want to make each other happy; they want to be there as confidants, helpers, and company. Friendship love is often compared to the love between siblings as well. In many cases, a sibling is just as good a friend as any. Society will not accept romantic love between relatives, but friendship love is encouraged and an amazing aspect to have. Everybody experiences love in a different way and may not agree with the concepts that it is dealt with. In the end, it does not matter whether or not the kind of love someone feels fits into any category. Love is personal, and the important thing is, is that it makes people happy and feels cared for. Whether people have romantic or friendship love, love is meant to mean that it is an emotion of a strong affection and personal attachment, and a virtue representing all of human kindness, compassion, and affection.

Ethics in Research The Tuskegee Syphilis Experiment Essay

The field of medicine has experienced rapid growth with in the few 150 to 200 years, and over the years we have learn that many of these scientific developments were made at the expense of unorthodox procedures and research carried out with little to no concern on the unethical aspects of the research, as medical science advance the researchers place little or no effort towards informing subjects about the nature of experiments. Tuskegee syphilis experiments in Alabama was on especially an infamous experiment, from ‘‘1932 to 1972’‘ the U.S. Public Health Services (PHS) conducted an experiment on 400 African American males in the late stages of syphilis these men, for the most part illiterate sharecroppers from one of the poorest counties in Alabama, were told what disease they were suffering from or of its seriousness, informed that they were being treated for â€Å"Bad Blood† their Doctors had no intention of curing them for syphilis at all. The data for the experiment was to be collected for autopsies of the men, and they were thus deliberately left to degenerate under the ravages of tertiary syphilis which can include tumors, heart disease, paralysis, blindness, insanity and death, this is the most unethical, immoral and genocidal act carried out and funded my U.S. Public Health Services. Coercion or leverage was the main tactic the PHS researchers use to recruit their test subjects, whom was poor uneducated sharecroppers that luck the resources needed to seek out side help, by misinforming the participants about the truth of their illness (syphilis), but non the less this was a means for the PHS to carry out their hidden agenda, to make them aware that they do have an illness and The U.S. Public Health Services was there to help treat them of their so call bad blood pandemic, this act of coercion provided the PHS the leverage they need to recruit necessary research subjects. Because of this the consent acquired by the researchers from the research subjects was invalid because the information provided by the researchers was subjects was invalid, this violation of inform consent stemmed further, the researchers did not explain the expectancy and severity of the research, the researchers deliberately did not inform the subjects  that they have the right to decline to participate and to withdraw from the research once it has begun. The spider web of lies and deception planed and carried out by The U.S. Public Health Services researchers had no plans of curing the syphilis pandemic, the deception in this research is the main underlying reason why this research study is unethical in nature, rather than the actual deception itself, the deliberate use of deception in this research resulted in the deaths of a 100 African Americans research subjects, these deaths was actually apart of the research it self. ‘’The study actively kept the men away from treatment, even after penicillin became widely used for the treatment of syphilis in 1950s. In February of 1969, the PHS decided to continue the  study and to continue to deny the men antibiotic treatment even though penicillin had become established as the standard treatment for syphilis, because it assumed that the disease was far too advanced for it to be treated. Associated Press reporter Jean Heller broke the story on July 25, 1972 on the front-page of the New York Times and the study stopped at once’’ After the research had ended the only seventy-four participants of 400 infected subjects had still been a live after the study got exposited, In August 1972, an ad hoc committee found the study to have been â€Å"ethically unjustified† and argued that penicillin should have been given to the men upon its discovery. 40 years after the experiment had ended the test subjects was not Adequate debriefed on the true nature of the study and the long-term effects of the of the syphilis on their bodies, wife and kids. After the press release of the Tuskegee syphilis experiment the test subjects was provided with little to no confidentiality from the press or PHS after the findings of the research was publish. In effect, I have been looking forward to building my own family the way I have been raised. Demureness was one virtue I always appreciated in my mother, one that in my own opinion, a real woman should possess so that this was the first thing I looked for in a woman. I guess in part, this is because of the fully accepted impression that men are expected to be strong as the head of the family, able to bring in the needs of the family by working difficult jobs while women are delicate, from their actions to their works. Armed with such a conviction, this has served as my guidance in finding the right woman for my self. I met my wife in _________ whose very refined movement caught my attention and whose cultured speech strengthened my desire to know her better. I soon learned she was looking for a man who wanted a lasting r elationship and not just one that would melt after a few years. Seeing it through the exchange theory (Adler & Proctor, 2007 p. 280), I now see how my wants were gratified as much as hers in the relationship we were trying to build. Our communication problems did not end when we got married rather we seem to have thought that each of us should be able to read one’s mind by this time because at this period, we should have known each other pretty well. This made the problem even worse which usually led to a cold war between us, my wife usually keeping quiet, not answering simple questions. The arrival of our children did not even help resolve our problems instead, they were involved in a tug of war where my wife and I tried to get their attention to one of us alone and let the other feel how it could be to be alone, abandoned. These days, there are still some misunderstandings but we have grown, being wiser in our actions and speech and reminded that if we truly want our relationship to last, we have to make things work. This includes learning to be articulate in communicating our thoughts, putting one’s self to the shoes of the other for us to understand further what one is saying. The subject matter we have been discussing in class gave me a clearer view of my relationship with my wife and is helping me a lot in the improvement of how I deal with her these days. I have learned that in a relationship, it sure takes two to tango and the resolution we tried to employ in our
